Community Physiotherapist

We are seeking a highly skilled and motivated Physiotherapist to join our Community Rehabilitation Team. As a key member of the team, you will be responsible for delivering high-quality rehabilitation services to patients in a community hospital setting.
Working closely with patients, families, and healthcare professionals, you will assess, plan, and implement individualized rehabilitation programs to promote optimal health and well-being.
With a strong focus on patient-centered care, you will work collaboratively with the multidisciplinary team to ensure seamless care transitions and effective discharge planning.
As a Band 5/6 Physiotherapist, you will have the opportunity to develop your clinical skills and expertise in a supportive and dynamic environment.

Key Responsibilities- Assess and plan individualized rehabilitation programs for patients
- Deliver high-quality rehabilitation services in a community hospital setting
- Work collaboratively with the multidisciplinary team to ensure seamless care transitions
- Develop and maintain strong relationships with patients, families, and healthcare professionals
- Contribute to the development and implementation of evidence-based practice guidelines
- Postgraduate degree in Physiotherapy or a related field
- Registration with the Health Professions Council
- Experience working in a community rehabilitation setting
- Strong communication and interpersonal skills
- Ability to work autonomously and as part of a team
