Community Physiotherapist

We are seeking a highly skilled and motivated Physiotherapist to join our Community Therapy Service as a Band 5/6/7 Community Physiotherapist. The successful candidate will be responsible for providing specialist assessment, treatment, and intervention to patients in their own homes and local community.
Key Responsibilities- Manage a caseload of patients in the community, including those with complex needs and disabilities.
- Provide specialist assessment, treatment, and intervention to patients in their own homes and local community.
- Work closely with the wider Multi-Disciplinary Team (MDT) and Integrated Neurosciences Team (INT) to ensure seamless care pathways.
- Develop and maintain effective relationships with patients, carers, and other healthcare professionals to ensure high-quality care.
- Contribute to the development and implementation of clinical guidelines and policies to improve patient outcomes.
The ideal candidate will have:
- Postgraduate therapy experience, including community and rehabilitation.
- Profession-specific diploma/degree in Occupational Therapy or Physiotherapy.
- Health Professions Council Registration.
- Evidence of Continuing Professional Development (CPD) maintained in a portfolio, including attendance at recent postgraduate courses relevant to clinical fields.
- Membership of the Chartered Society of Physiotherapy/Royal College of Occupational Therapy.
We offer a supportive and enthusiastic team environment, with opportunities for professional development and growth. Our team comprises qualified physiotherapists, assistant practitioners, and assistant therapy staff, who work closely with the wider MDT and INT teams in the community.
We have appointed a new Trainee Advanced Clinical Practitioner in community and there is an opportunity to develop an interest in frailty and long-term conditions.
6-day working is in effect - Monday to Saturday and bank holidays on a rota basis.
