About the Role

We are seeking a highly skilled and experienced Physiotherapist to join our Community Therapy Service as a Physiotherapist, working across community delivering rehabilitation in patients' own homes following discharge from an acute bed, or supporting Long term condition management.

Key Responsibilities
  • Assess and treat patients with a variety of complex conditions, including long term condition management and acute hospital discharges.
  • Manage a caseload of patients and provide specialist assessment and treatment, including the prescription of essential equipment as part of the therapeutic intervention.
  • Work closely with the wider MDT and INT teams in community, including PCN and MDT teams, to provide seamless care.
  • Develop and maintain a high level of clinical expertise, staying up-to-date with the latest research and best practices in community rehabilitation.
