Job Summary

An exciting opportunity has arisen for a community rehabilitation specialist to join our supportive and enthusiastic community therapy service as a Physiotherapist, working across community delivering rehabilitation in patients' own homes following discharge from an acute bed, or supporting long-term condition management.

This is a maternity cover for 12 months.

The team comprises of qualified physiotherapists, and assistant practitioners, and assistant therapy staff, who work closely with the wider multidisciplinary team and internal teams in community.

We have appointed a new Trainee Advanced Clinical Practitioner in community and there is an opportunity to develop an interest in frailty and long-term conditions.

You will be working with patients with a large variety of conditions and disabilities, in their own homes and local community. You will be responsible for assessment, intervention, and discharge of a caseload of patients, act as the direct supervisor to staff and provide support and expertise to the other members of the team.

Main Duties of the Job

The post holder is an autonomous practitioner, responsible for the provision of specialized assessment and treatment of patients referred to the service. Responsible for managing a caseload of patients with a variety of complex conditions, including long-term condition management and acute hospital discharges. You will see patients in their own homes, working with a multidisciplinary team in other organizations. You will carry out specialist assessment and treatment, including the prescription of essential equipment as part of the therapeutic intervention.

You will play an important link with Primary Care Networks and multidisciplinary teams in attending meetings as part of neighborhood team working.

The service operates across 6 days.

Part-time working and secondment will be considered.

Person Specification

EVIDENCE OF

Essential

  • Postgraduate therapy experience to include community and rehabilitation.
  • Profession-specific Diploma/degree in Occupational Therapy or Physiotherapy.
  • Health Professions Council Registration.
  • Evidence of Continuing Professional Development maintained in a portfolio, including attendance at recent postgraduate courses relevant to the clinical fields in rotations.
  • Membership of the Chartered Society of Physiotherapy/Royal College of Occupational Therapy.
  • Involvement in Special Clinical Interest Group.
