Care Navigator

We are seeking a highly motivated and organized Care Navigator to join our team at UWE Frenchay Health Centre. As a key member of our reception team, you will be the first point of contact for our students and patients, providing support and guidance on choosing the right service at the right time.
Key Responsibilities- Maintain and monitor the practice appointment system, ensuring efficient and effective management of patient bookings.
- Process personal, telephone, and e-requests for appointments, prioritizing urgent cases and directing patients accordingly.
- Provide exceptional customer service, being friendly and welcoming to all students and patients, and remaining calm and professional in challenging situations.
- Answer incoming telephone calls, transferring calls or dealing with callers' requests appropriately, and understand how to prioritize patients in need of urgent medical attention.
- Deal with requests from students, patients, and clinical team members for emergency calls, following established protocols.
- Signpost students and patients to correct services, and initiate contact with and respond to requests from students, patients, team members, and external agencies.
- Monitor and maintain the reception area, ensuring a clean and welcoming environment for patients.
- Support all clinical staff with general tasks as requested, and assist with any other duties necessary to ensure the smooth running of the practice.
- Essential:
- GCSE level or equivalent education.
- Previous experience of working with people.
- Desirable:
- Previous experience of working in Primary Care.
UWE Frenchay Health Centre is a modern campus with great shops and restaurants on site. We are close to Abbey wood retail park and Bristol Parkway station, with excellent transport links.
