Position Overview

Join the Black Country Healthcare NHS Foundation Trust during a pivotal moment in mental health services. We are launching a new initiative designed collaboratively with young individuals aged 18-25, aimed at enhancing support across health, social care, and community sectors for young adults in our region.

This service will complement existing offerings, creating a comprehensive support system. Our model includes pathways to facilitate transitions between Child and Adolescent Mental Health Services (CAMHS) and Adult Mental Health Services (AMHS), providing enhanced support for young adults from underrepresented groups, and establishing a robust array of community resources to promote well-being for young people and their significant relationships.

The program is designed to be needs-based, offering choices and promoting inclusivity. About the Role

As a Youth Transition Support Specialist, you will adopt and advocate for an empowering, relational, and trauma-informed approach in all interactions. You will engage with young individuals on a one-on-one basis and participate in joint appointments with CAMHS and AMHS to facilitate transitions and access to various community resources. You will initially work within one of several available pathways, contributing to your local 'young person hub' and collaborating with social care and the voluntary sector to support disadvantaged young adults seeking additional assistance.

Locations Available

This role will primarily involve face-to-face interactions rather than remote work.

Essential Qualifications

  • Professional registration is required for eligibility (e.g., Occupational Therapist, Social Worker, Speech and Language Therapist, or other Allied Health Professionals).
  • Experience working with individuals in significant distress.
  • Familiarity with at least one psychological approach.
  • Ability to work in an empowering and adaptable manner.

Preferred Qualifications

  • Strong skills in engaging young people, potentially including interests and abilities in art, music, or other creative activities.
  • Experience in a supportive role with disadvantaged groups, such as individuals who have been in care, the homeless, those who have faced significant trauma or adversity, individuals from minority cultural backgrounds, or those with disabilities or substance misuse issues.
