Main ResponsibilitiesThe successful candidate will be responsible for providing expert advice and support on medicines optimisation, working closely with our Integrated Care Pharmacist to ensure seamless pathways for medication support. You will work independently to provide a seamless pathway for medication support from in-patient to community services, developing prioritisation matrices, referral pathways, and undertaking structured medication reviews. You will also support the development of a comprehensive medicines optimisation service, working across health and social care interfaces to reduce the risk of harm from taking medicines and support procedures for medicines reconciliation, adherence, and self-administration.
About the RoleThis is an exciting opportunity to join our team and contribute to the development of a truly integrated care framework across health and social care services. You will work closely with our multidisciplinary team to ensure the safe, effective, and economical use of medicines, supporting the development of a comprehensive medicines optimisation service. You will also have the opportunity to develop partnerships and facilitate collaborative working between multidisciplinary teams, especially during the transfer of care to optimise medicine-related discharge processes.
Person SpecificationWe are looking for a motivated and dynamic individual with excellent interpersonal and IT skills. You will have a BTEC and NVQ3 (or equivalent) in Pharmaceutical Science and be registered as a Pharmacy Technician with the General Pharmaceutical Council. You will also have experience in a broad range of hospital/community pharmacy activities, including dispensary management and clinical pharmacy activities. Nationally recognised qualifications as an Accredited Checking Technician and Medicines Management Technician are desirable.
