Are you an enthusiastic Pharmacist seeking a rewarding opportunity in the field of Medicines Management?

Our team at HCRG Care Group is in search of a committed Pharmacist to join us on a part-time basis (20 hours per week) to embark on a significant role in the area of Medicines Optimisation.

The Medicines Management team provides essential services aimed at enhancing care for individuals in community and social health environments. This includes a variety of settings such as community hospitals and home care services, where you will play a crucial role in advancing medicines optimisation, governance, and safety within the pharmacy team.

Your primary location will be at our designated healthcare facility, but travel to various sites will be a requirement of this position. Therefore, possessing a valid driving license and access to a vehicle is essential.

We encourage applications from pharmacists with experience in both hospital and primary care sectors, including community pharmacy.

Main Responsibilities

As a Medicines Optimisation Pharmacist, you will be integral in promoting effective medicines use and ensuring high-quality, safe prescribing practices across our service areas. Your responsibilities will include:

  • Providing expert advice on prescribing, therapeutics, and medicines governance.
  • Collaborating with clinical pharmacists on inpatient wards and community healthcare teams to enhance medicines safety and optimisation.
  • Assisting the Lead Pharmacist in ensuring compliance with the Home Office Controlled Drugs License.
  • Ensuring adherence to legal and regulatory frameworks related to medicines management, including NICE and MHRA guidelines.
  • Conducting and facilitating audits to evaluate and improve medicines management systems.
  • Supporting quality initiatives to educate and inform colleagues about safety and governance.
  • Leading the development and review of medicines management policies and procedures.

  • Registration with the General Pharmaceutical Council.
  • Evidence of continuous professional development.
  • Experience in hospital or community pharmacy settings.
  • Understanding of clinical governance and quality improvement.
  • Ability to analyze and interpret data effectively.
  • A valid driving license and access to a vehicle for work-related travel.
  • Experience in using audits to drive service improvements.
  • Knowledge of medicines-related policy development.
