Care Coordinator Opportunity

A leading supplier of advocacy services is seeking a skilled Care Coordinator to join their team on a fixed-term contract. This fully remote role will support regional team leaders with various duties, including coordination of clients and referrals, managing referral waiting lists, and processing sensitive client information.

Key Responsibilities:
  • Coordinate client interactions and referrals
  • Manage and prioritize referral waiting lists
  • Process sensitive client information and liaise with social workers
  • Communicate with clients and third parties via email and phone
Requirements:
  • Strong communication skills and ability to provide support over phone and email
  • Confident working with data and computer literate
  • A personable nature and ability to interact with vulnerable individuals
  • Ability to prioritize and make decisions
  • Experience in advocacy, social care, or a related field is advantageous but not essential
