Job Summary

We are seeking a skilled and compassionate Community Reablement Worker to join our team. As a key member of our Reablement Service, you will work closely with individuals in their own homes to help them regain their independence and confidence. Your expertise will be invaluable in supporting people to relearn essential skills and adapt to changing circumstances.

Key Responsibilities

  • Work collaboratively with individuals, families, and carers to develop personalized support plans
  • Provide practical assistance and guidance to help individuals regain independence in daily living tasks
  • Conduct assessments and reviews to identify individual needs and goals
  • Develop and maintain effective relationships with healthcare professionals, social services, and other stakeholders
  • Participate in team meetings and contribute to service development and improvement initiatives
