Position Overview

We are seeking a dedicated Clinical Nurse Specialist to join our Gastroenterology team. This role will primarily focus on supporting the Lead Consultant for upper gastrointestinal referrals. The successful candidate will play a vital role in managing patient pathways, from initial referral through to diagnosis and treatment for individuals experiencing unexplained gastrointestinal symptoms.

Key Responsibilities

  • Act as the primary contact for patients referred to the upper GI service, guiding them and their families through the care pathway.
  • Manage electronic patient referrals and ensure timely consultant reviews.
  • Collaborate with the Lead Consultant to assess new patient referrals and establish an effective management plan.
  • Identify and address the physical, social, and psychological needs of patients with suspected gastrointestinal conditions.

Management Duties

  • Collect and analyze data related to patient pathways, tracking key events and clinical outcomes.
  • Present findings to professional groups and provide written reports to identify risks or delays in patient care.
  • Maintain effective communication with relevant personnel and departments to promote collaborative working relationships.
  • Coordinate follow-up appointments and ensure that essential monitoring is conducted.

Clinical Responsibilities

  • Provide expert clinical knowledge and support to patients and their families throughout their care journey.
  • Ensure patients are well-informed about the purpose and outcomes of investigations, adhering to best practices.
  • Facilitate the preparation of patients for investigations and ensure a smooth discharge process.
  • Communicate complex information effectively to patients, their families, and the multidisciplinary team.

Education and Leadership

  • Deliver high-quality patient care while evaluating services for continuous improvement.
  • Participate in multidisciplinary team meetings and contribute to the development of service objectives.
  • Act as a patient advocate, ensuring that all relevant information is documented accurately in patient records.
  • Support and mentor nursing staff, fostering an environment of professional development.
