Operating independently, the practitioner will be responsible for the continuous assessment and management of patients, acting as a clinical mentor and resource for other staff members. Detailed Responsibilities
- Conduct comprehensive ward rounds alongside the General Practitioner and nursing leadership.
- Manage high-acuity patients autonomously, demonstrating advanced clinical judgment.
- Prescribe medications while effectively linking pharmacological knowledge to patient care.
- Respond to medical emergencies, providing support to nursing staff as needed.
- Make informed, evidence-based decisions in collaboration with patients and healthcare professionals.
- Lead daily multidisciplinary team meetings, ensuring effective communication and care coordination.
- Document and finalize discharge summaries accurately.
- Advocate for the role of advanced clinical practitioners and support the development of trainee practitioners.
- Drive improvements in care pathways through evidence-based practices.
- Contribute to workforce transformation initiatives within the organization.
Candidate Profile
Essential Criteria
- Proven experience working autonomously as an advanced clinical practitioner.
- Understanding of primary care's role in supporting acute care transitions.
- Ability to assess and identify patients at risk of deterioration.
Desirable Criteria
- Minimum of two years of experience within an NHS setting.
- Background in emergency care or acute assessment units is advantageous.
- Experience in team management and staff retention strategies.
