The post holder will be employed by North Middlesex University Hospital (Enfield Community) with primary responsibility at Chase Farm Hospital on Cape Town and Canterbury wards. Occasionally, the post holder may be expected to work across the wider team at Magnolia Unit and Virtual ward.
Main DutiesThe post holder will have continuing responsibility for proactively monitoring and managing patients on the ward, acting as a clinical leader and role model to staff within the service, offering clinical advice on the care of patients with complex needs as required. You will work with the GP, colleagues, and managers to support the integration of care pathways and patient care across community settings.
Key Responsibilities- Assess and manage high acuity complex patients autonomously
- Prescribe medication and link pharmacology to patients' treatment and presentation
- Respond appropriately to medical emergencies and support the nursing team
- Make advanced, evidence-based decisions, in partnership with patients and other professionals, using agreed protocols
- Work closely with the GP leading daily MDT meetings
- Update and complete discharge summaries
Essential Criteria
- Experience of working autonomously and independently as an ACP
- Understanding of the role of Primary care in supporting acute wards to step down patients and supporting a timely discharge
- Ability to assess and identify deteriorating patients and recognize early symptoms of disease
Desirable Criteria
- 2 years' experience within an NHS organization
- Experience in A&E, Acute assessment units, or similar settings
- Experience of managing staff recruitment and retention
- Experience of managing a team
