Job overview

The post holder will be employed by North Middlesex University Hospital (Enfield Community) with primary responsibility at Chase Farm Hospital on Cape Town and Canterbury wards. Occasionally the post holder may be expected to work across the wider team at Magnolia Unit and Virtual ward.

Patients will be identified as being medically optimised but requires community support while waiting for either placement or package of care or requiring rehabilitation.

Working autonomously, the post holder will have continuing responsibility for proactively monitoring and managing, patients on the wardand act as a clinical leader and role model to staff within the service, offering clinical advice on the care of patients with complex needs as required. You will work with the GP, colleagues and managers to support the integration of care pathways and patient care across community settings.

Main duties of the job

You will be expected to have skills in advanced physical assessment, diagnostic reasoning, nurse independent prescribing and managing patient with complex needs. Using these skills will assist in the recognition of early symptoms of disease exacerbation, acute illness and injuries. 

The post holder will co-ordinate care and navigate the patient through pathways across the primary, secondary care interface, including the prevention of unnecessary readmissions, facilitating appropriate ones through in-reach case finding from medical /surgical wards at NMUH and key partners North London (Barnet, Whittington, Royal Free hospital, UCLH for rehab patients for rehabilitation.

Detailed job description and main responsibilities

Carry out ward rounds at the beginning of each shift with the General Practitioner (GP), Matron and Ward Manager

Be able to carry out a ward round in the absence of a GP

Assessing and managing high acuity complex patients autonomously

Prescribing medication and ability to link pharmacology to patients' treatment and presentation

Respond appropriately to medical emergencies and support the nursing team.

 Make advanced, evidence-based decisions, in partnership with patients and other professionals, using agreed protocols, but also where there are no appropriate precedents.

Work closely with the GP leading daily MDT meeting.

Update and complete discharge summaries.

Champion the role of ACPs and supporting nurses, Trainee ACPs find their feet in order to craft for the next generation to benefit the complex needs of the local population.

Help drive improvements across pathways, showing evidence-based practice.

 Help drive transformation of the workforce within the organization.

Essential criteria

Experience of working autonomously and independently as an ACP Understanding of the role of Primary care in supporting acute wards to step down patients and supporting a timely discharge Ability to assess and identify deteriorating patients and recognize early symptoms of disease

Desirable criteria

2 years experience within NHS organization Experience in A&E, Acute assessment units, similar setting would be advantageous Experience of managing staff recruitment and retention. Experience of managing a team
