Job Overview
The post holder will be employed by North Middlesex University Hospital with primary responsibility at Chase Farm Hospital. The role involves working autonomously to monitor and manage patients on the ward, acting as a clinical leader and role model to staff, and offering clinical advice on patient care.
Main Duties
The post holder will be expected to have skills in advanced physical assessment, diagnostic reasoning, nurse independent prescribing, and managing patients with complex needs. They will coordinate care and navigate patients through pathways across the primary and secondary care interface, preventing unnecessary readmissions and facilitating appropriate ones.

Key Responsibilities
- Carry out ward rounds with the General Practitioner, Matron, and Ward Manager
- Assess and manage high acuity complex patients autonomously
- Prescribe medication and link pharmacology to patient treatment and presentation
- Respond to medical emergencies and support the nursing team
- Make advanced, evidence-based decisions in partnership with patients and other professionals
Person Specification
Essential Criteria
- Experience of working autonomously and independently as an Advanced Clinical Practitioner
- Understanding of the role of Primary care in supporting acute wards to step down patients and support timely discharge
- Ability to assess and identify deteriorating patients and recognize early symptoms of disease
Desirable Criteria
- 2 years' experience within an NHS organization
- Experience in A&E, Acute Assessment Units, or similar settings
- Experience of managing staff recruitment and retention
- Experience of managing a team
