Detailed Job Description and Responsibilities
- Conduct regular ward rounds with the GP and other healthcare leaders.
- Manage high-acuity patients autonomously.
- Prescribe medications, linking pharmacological knowledge to patient care.
- Respond effectively to medical emergencies and support nursing staff.
- Make informed, evidence-based decisions in collaboration with patients and other healthcare professionals.
- Lead daily multidisciplinary team meetings with the GP.
- Complete and update discharge summaries as necessary.
- Advocate for the role of Advanced Clinical Practitioners and support the development of new practitioners.
- Drive improvements in care pathways through evidence-based practices.
- Contribute to workforce transformation initiatives within the organization.
Person Specification
Essential Criteria
- Proven experience working autonomously as an Advanced Clinical Practitioner.
- Understanding of primary care's role in supporting acute care transitions.
- Ability to assess and recognize deteriorating patient conditions.
Desirable Criteria
- Two years of experience within a healthcare organization.
