Position Title: Community Support Coordinator

Job Overview

Service Care Solutions is in search of a dedicated and experienced Community Support Coordinator to become a vital member of our Community team.

In this role, you will be instrumental in orchestrating care and assistance for individuals (along with their caregivers and families) who reach out or are directed to adult social services from the community.

You will serve as the primary community-facing access point for inquiries related to adult social services and safeguarding issues.

Key Responsibilities


1. Manage a diverse caseload while collaborating with professionals across the team to ensure a cohesive service delivery to individuals with varying needs.

2. Execute a variety of social care functions including screening and triage, initial evaluations, support planning, and case management to the highest standards.

3. Assist individuals in recognizing their strengths and support networks, aiming to delay or prevent the necessity for new or additional social care interventions, thereby enabling them to maintain their preferred lifestyle.

4. Demonstrate exemplary practices in engaging with adults in a strengths-based and person-centered manner, linking individuals with informal, community-based resources whenever feasible.

5. Fully commit to collaborating with residents, caregivers, community organizations, and other essential professionals to empower individuals to achieve maximum independence.


Qualifications

• Proven experience working with vulnerable adults and a solid understanding of adult safeguarding principles.
• Exceptional communication and interpersonal abilities.
• Capability to thrive in a dynamic environment while managing multiple priorities effectively.
