We C.A.R.E. Home Health Agency, LLC is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our team on a part-time and per diem basis.

As an LPN, you will play a vital role in providing exceptional in-home healthcare services to adults. Your responsibilities will encompass a range of nursing duties, including wound care, infusion services, and medication education. You will also be responsible for meticulous documentation of all patient interactions and care provided within our electronic health record system.

Key Responsibilities:

  • Deliver personalized in-home nursing care to adults, encompassing wound care, infusion services, medication administration, and education.
  • Maintain accurate and detailed records of all patient visits and care provided within our electronic health record system.
  • Provide guidance and supervision to non-skilled staff members who provide direct patient care in the home setting.

This role offers a flexible schedule with minimal time spent in the office. You will primarily work in the field, providing direct patient care within their homes.

Qualifications:

  • Graduate from an accredited LPN/LVN School of Nursing.
  • Current and active Licensed Practical Nurse (LPN) license in the state of Pennsylvania.
  • One or more years of experience in a community/home health agency or home setting is preferred but not required.
  • Strong attention to detail and commitment to providing high-quality patient care.
  • Current CPR certification (online certifications are not accepted).
  • Successful completion of a criminal background check.
  • Valid driver's license with current active auto insurance.
