About Us
Collingwood Health Group is a large cohesive team serving a population of 25,000 over 3 sites. We have a team of 20 doctors, 7 nurses, 1 nursing assistant, 2 healthcare assistants, and a phlebotomist. Our nursing team has an overall lead and meets regularly as a nursing team as well as meeting with the whole clinical team.
Job Summary
We are seeking a skilled and experienced practice nurse to join our nursing team. The successful candidate will provide patient-facing nursing services to our practice population, including chronic disease management, women's health, vaccination and immunization programs, and general practice nursing duties.
Main Duties
The main duties of the role will include:
- Providing patient-facing nursing services to the practice population
- Managing chronic disease management, including diabetes, respiratory, hypertension, and cardiovascular disease
- Providing women's health services, including screening and vaccinations
- Delivering vaccination and immunization programs, including seasonal vaccinations
- Performing general practice nursing duties, including injections, vaccinations, health promotion, and chronic disease management
About the Role
The successful candidate will have experience working in a general practice setting and will be able to work independently within the practice team. They will be responsible for managing their own clinical practice and that of other members of the nursing team to ensure safe and effective care. The candidate will also be responsible for accepting delegated responsibility and inherent accountability for the quality of their practice.

The ideal candidate will have:
- Experience of working in a general practice setting
- Ability to work independently within the practice team
- Strong clinical skills and experience in chronic disease management, women's health, and vaccination and immunization programs
- Excellent communication and interpersonal skills
- Ability to work as part of a team and contribute to the development of the nursing team
