Main Responsibilities- Provide patient-facing nursing services to the practice population, including chronic disease management, women's health, and vaccination programs.
- Assess, plan, and evaluate clinical nursing care to meet the needs of individuals and groups.
- Take the clinical lead in defined areas of nursing practice within the primary healthcare team.
- Monitor work areas and practices to ensure they are safe and compliant with legislation.
- Receive direct referrals from healthcare professionals, other agencies, and patients.
- Plan, manage, and deliver programs to improve health and disease in individuals or groups.
