About the Role
We are seeking a skilled and dedicated Practice Nurse to join our team at Collingwood Health Group. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the overall success of our practice.
Key Responsibilities
- Provide patient-facing nursing services to our practice population, including chronic disease management, women's health, and vaccination programs.
- Assess, plan, and evaluate clinical nursing care to meet the needs of individuals and groups.
- Take the clinical lead in defined areas of nursing practice within the primary healthcare team.
- Monitor and maintain a safe and healthy work environment, adhering to safety legislation and protocols.
- Collaborate with healthcare professionals and other agencies to deliver comprehensive patient care.
Person Specification
We are looking for a candidate with:
Experience working in a general practice setting, preferably with a focus on respiratory medicine.Strong communication and interpersonal skills, with the ability to work effectively in a team environment.Excellent clinical skills and knowledge, with a commitment to ongoing professional development.What We Offer
