Job Summary: We are seeking a skilled Registered Nurse to join our team as a Per Diem Nurse Care Coordinator. The ideal candidate will have a strong background in home health care and be able to provide comprehensive care to patients in their homes.

Responsibilities:

* Coordinate total patient care by conducting comprehensive health and psychosocial assessments in the home setting.
* Perform admission, transfer, re-certification, resumption of care, and discharge of the home care patient.
* Provide oversight for non-skilled staff members providing direct care to recipients in the home setting.
* Must be willing to provide in-home nursing care (wound care, infusion services, medication education) to adults and manage patients.
* Role is primarily field based with minimal time spent in the office.
* Must be a graduate from an accredited School of Nursing.
* Must be licensed in the state of Pennsylvania, as a Registered Nurse.
* One, or more, years of experience, in a community/home health agency or in a home setting, is preferred.
* Must have knowledge of Medicare and Medicaid guidelines.
* Must be detail oriented.
* Must have a criminal background check.
* Must have current CPR certification. Online certification is not acceptable.
* Must have a valid driver's license with current active auto insurance.

Requirements:

* Graduate from an accredited School of Nursing
* Licensed in the state of Pennsylvania as a Registered Nurse
* One, or more, years of experience in a community/home health agency or in a home setting
* Knowledge of Medicare and Medicaid guidelines
* Detail oriented
* Current CPR certification
* Valid driver's license with current active auto insurance
