We are seeking a skilled and compassionate Registered Nurse to join our team at We C.A.R.E. Home Health Agency, LLC. As a Registered Nurse, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Job Summary:

As a Registered Nurse with our agency, you will be responsible for conducting comprehensive health and psychosocial assessments, developing and implementing individualized care plans, and providing oversight to non-skilled staff members. You will also be responsible for performing admission, transfer, re-certification, resumption of care, and discharge of home care patients.

Responsibilities:
  • Coordinate total patient care by conducting comprehensive health and psychosocial assessments in the home setting.
  • Perform admission, transfer, re-certification, resumption of care, and discharge of the home care patient.
  • Provide oversight for non-skilled staff members providing direct care to recipients in the home setting.
  • Must be willing to provide in-home nursing care (wound care, infusion services, medication education) to adults and manage patients.
  • Role is primarily field-based with minimal time spent in the office.
  • Must be a graduate from an accredited School of Nursing.
  • Must be licensed in the state of Pennsylvania, as a Registered Nurse.
  • One, or more, years of experience, in a community/home health agency or in a home setting, is preferred.
  • Must have knowledge of Medicare and Medicaid guidelines.
  • Must be detail-oriented.
  • Must have a criminal background check.
  • Must have current CPR certification. Online certification is not acceptable.
  • Must have a valid driver's license with current active auto insurance.
