Job Summary:The Registered Nurse Per Diem will be responsible for coordinating total patient care, conducting comprehensive health and psychosocial assessments, and providing oversight for non-skilled staff members. This role requires a graduate from an accredited School of Nursing, licensure in the state of Pennsylvania, and one or more years of experience in a community/home health agency or home setting.
Responsibilities:- Conduct comprehensive health and psychosocial assessments in the home setting.
- Perform admission, transfer, re-certification, resumption of care, and discharge of the home care patient.
- Provide oversight for non-skilled staff members providing direct care to recipients in the home setting.
- Manage patients and provide in-home nursing care, including wound care, infusion services, and medication education.
- Role is primarily field-based with minimal time spent in the office.
- Must be detail-oriented and have knowledge of Medicare and Medicaid guidelines.
- Must have a valid driver's license with current active auto insurance.
- Graduate from an accredited School of Nursing.
- Licensed in the state of Pennsylvania as a Registered Nurse.
- One or more years of experience in a community/home health agency or home setting.
- Knowledge of Medicare and Medicaid guidelines.
- Detail-oriented and able to work independently.
