We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at We C.A.R.E. Home Health Agency, LLC. As a LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities:

  • Provide in-home nursing care services, including wound care, infusion services, and medication education, to adults in the home setting.
  • Document all visits in the electronic record with detailed nursing notes to ensure accurate and comprehensive care.
  • Oversee non-skilled staff members providing direct care to recipients in the home setting, ensuring seamless and efficient care delivery.
  • Work primarily in the field with minimal time spent in the office, with a flexible schedule to accommodate patient needs.
  • Collaborate with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Participate in ongoing education and training to stay up-to-date on the latest best practices and industry developments.
  • Meet the physical and emotional demands of the role, including lifting, bending, and standing for extended periods.

Requirements:

  • Graduate from an accredited LPN/LVN School of Nursing.
  • Licensed in the state of Pennsylvania as an LPN.
  • One or more years of experience in a community/home health agency or in a home setting, preferred but not required.
  • Detail-oriented and able to maintain accurate records and documentation.
  • Current CPR certification, with online certification not accepted.
  • Physical and TB test required.
  • Valid driver's license with current active auto insurance.
