We C.A.R.E. Home Health Agency, LLC is seeking a compassionate and dedicated Registered Nurse (RN) to join our team on a per diem basis. This role offers the opportunity to make a real difference in the lives of patients within their homes.

As a Registered Nurse, you will provide comprehensive nursing care, including assessments, wound care, infusion services, medication education, and patient management. You will also oversee non-skilled staff members providing direct care to recipients in the home setting.

Responsibilities:
  • Conduct thorough health and psychosocial assessments of patients in their homes.
  • Manage patient admissions, transfers, re-certifications, resumption of care, and discharges.
  • Supervise non-skilled staff members providing direct patient care.
  • Deliver a range of nursing services, including wound care, infusion therapy, medication education, and overall patient management.
  • Maintain accurate documentation and adhere to established protocols and guidelines.
Qualifications:
  • Graduate from an accredited School of Nursing.
  • Current Registered Nurse (RN) license in the state of Pennsylvania.
  • One or more years of experience in a community/home health agency or home setting is preferred.
  • Knowledge of Medicare and Medicaid guidelines.
  • Strong attention to detail and excellent communication skills.
  • Current CPR certification (online certification not accepted).
  • Valid driver's license with current active auto insurance.
