Job Summary

We are seeking a highly skilled and experienced Community Specialist Practitioner to join our team at Manchester University NHS Foundation Trust. As a key member of our Neighbourhood team, you will be responsible for leading and coordinating nursing teams to provide complex nursing care to adults in their own homes and various community settings.

You will work closely with multidisciplinary teams, including social workers, GPs, and allied health professionals, to ensure effective patient care coordination and demonstrate a positive impact on our patients. This is an exciting opportunity to develop new and existing clinical skills, as well as access a range of training opportunities for personal development.

Main Responsibilities
  • Lead and coordinate nursing teams to provide complex nursing care to adults in their own homes and community settings
  • Work closely with multidisciplinary teams to ensure effective patient care coordination
  • Develop and implement clinical skills and knowledge to improve patient outcomes
  • Collaborate with colleagues to develop and implement care plans that meet the needs of patients
  • Contribute to the development of the Community Nursing Service and identify opportunities for improvement
Requirements
  • Registered Nurse qualification
  • Community Specialist Practitioner qualification
  • Experience of working in community nursing settings
  • Ability to work autonomously and as part of a team
  • Excellent communication and interpersonal skills
