About the Role

ABL Health Ltd is seeking a highly motivated and experienced Health and Wellness Program Coordinator to join our team. As a key member of our team, you will be responsible for delivering high-quality, evidence-based physical activity and exercise services to adults aged 18 and over.

Key Responsibilities
  1. Program Delivery: Deliver ABL Health's adult weight management program, including nutrition, behavior change, and exercise delivery to groups and through one-to-one sessions, in the community and through digital methods.
  2. Exercise Programmes: Produce individual and detailed exercise programs, considering special adaptations required to accommodate overweight, obese, and physically inactive patients.
  3. Physical Activity and Exercise Advice: Translate physical activity and exercise information into a format that is appropriate and understandable for patients to support compliance with increased levels of physical activity and reduced sedentary behavior.
  4. Special Populations: Have sound knowledge of prescribing exercise and advising on physical activity to special populations, specifically obesity or underweight, as well as at population/community level.
  5. Weight Management and Nutrition: Have knowledge of specific physical activity and exercise requirements for delivering appropriate weight management programs for adults over 18 years of age.
  6. Client Progress Monitoring: Monitor clients' progress through regular consultation and with the use of Microsoft Excel, identifying specific treatment for those not on track to achieve their goals.
  7. Administrative Tasks: Keep detailed files, recording data accurately, ensuring all forms and other documentation is completed correctly.
  8. Professional Development: Have a thorough understanding of ABL Health policies and procedures and complete mandatory training and any additional training needed as part of the job role.
  9. Community Links and Partnership Working: Build community links and partnership working to encourage new referrals into the service and maintain close working relationships with other community services.
  10. Service Presentation: Present service and referral pathway information at stakeholder meetings as directed by the project lead.
  11. Clinical Governance: Work within the framework of clinical governance to maintain and improve patient care, quality, and efficiency and demonstrate personal and professional development in line with the ethos of lifelong learning.
Requirements
  • Experience in delivering specialist exercise programs and a background in nutrition, exercise, public health, community health, or adult and/or family weight management.
  • Excellent team player with a willingness to learn and develop.
  • Ability to translate physical activity and exercise information into a format that is appropriate and understandable for patients.
  • Sound knowledge of prescribing exercise and advising on physical activity to special populations.
  • Knowledge of specific physical activity and exercise requirements for delivering appropriate weight management programs.
  • Ability to work within the framework of clinical governance to maintain and improve patient care, quality, and efficiency.
