About the Role

We are seeking a highly skilled and compassionate Care Co-ordinator to join our team at C M Community Care Limited. As a Care Co-ordinator, you will play a vital role in ensuring the delivery of high-quality care services to our clients.

Key Responsibilities
  • Assist in the creation of rotas and working patterns for all staff to ensure all service is covered with the support of a Care Manager.
  • Coordinate service delivery daily, managing inquiries from staff, clients, and other health professionals we work alongside.
  • Undertake formal checks on staff in the field.
  • Undertake initial care assessments and create and update client care plans.
  • Contribute towards the delivery of a high standard of care through effective communication and teamwork.
  • Provide cover delivering care in contingency.
Requirements
  • Must have a full UK driving license and car.
  • Must have a smartphone.

  • Excellent communication and teamwork skills.
  • Experience in this type of role is preferred.
