Care Coordinator

We are seeking a highly skilled and compassionate Care Coordinator to join our team at C M Community Care Limited. As a Care Coordinator, you will play a vital role in ensuring the delivery of high-quality care services to our clients. Your primary responsibility will be to coordinate the daily operations of our care services, including managing inquiries from staff, clients, and other health professionals. You will also be responsible for undertaking formal checks on staff in the field, conducting initial care assessments, and creating and updating client care plans. Additionally, you will contribute to the delivery of a high standard of care through effective communication and teamwork. Key Responsibilities
- Coordinate the daily operations of our care services, including managing inquiries from staff, clients, and other health professionals.
- Undertake formal checks on staff in the field.
- Conduct initial care assessments and create and update client care plans.
- Contribute to the delivery of a high standard of care through effective communication and teamwork.
Requirements
- Must have a full UK driving license and car.
- Must have a smartphone.
- Excellent communication and teamwork skills.
- Experience in a care setting is preferred.
