Job DescriptionWe are seeking a highly organized and communicative Care Coordinator to join our team. The successful candidate will be responsible for coordinating service delivery, managing inquiries from staff, clients, and other health professionals, and undertaking formal checks on staff in the field.
The ideal candidate will have a natural caring approach to life, excellent communication and teamwork skills, and experience in a care setting. They will be able to work independently and as part of a team, and will be committed to delivering high-quality care to our clients.
Responsibilities- Assist with the creation of rotas and working patterns for all staff to ensure all service is covered.
- Coordinate service delivery daily, including managing inquiries from staff, clients, and other health professionals.
- Undertake formal checks on staff in the field.
- Undertake initial care assessments and create and update client care plans.
- Contribute towards the delivery of a high standard of care through effective communication and teamwork.
- Provide cover delivering care in contingency.
- Must have a full UK driving license and car.
- Must have a smartphone.
- Must have a natural caring approach to life.
