About the Role

We are seeking a highly skilled and compassionate Care Co-ordinator to join our team at C M Community Care Limited. As a Care Co-ordinator, you will play a vital role in ensuring the delivery of high-quality care services to our clients.

Key Responsibilities
  • Assist in the creation of rotas and working patterns for all staff to ensure seamless service delivery.
  • Coordinate daily service delivery, managing inquiries from staff, clients, and other healthcare professionals.
  • Conduct formal checks on staff in the field.
  • Undertake initial care assessments and create and update client care plans.
  • Contribute to delivering exceptional care through effective communication and teamwork.
  • Provide cover for care delivery in contingency situations.
Requirements
  • Must have a full UK driving license and car.
  • Must have a smartphone.
  • Must have a natural caring approach to life.
  • Excellent communication and teamwork skills.
  • Experience in a care setting is preferred.
