We are seeking a skilled and compassionate individual to fill the role of Care Co-ordinator, joining our team of dedicated professionals at C M Community Care Limited. The Care Co-ordinator will be responsible for coordinating service delivery, managing inquiries from staff, clients, and other health professionals, and undertaking formal checks on staff in the field. This role also involves undertaking initial care assessments, creating and updating client care plans, and contributing towards the delivery of a high standard of care through effective communication and teamwork.

Responsibilities
  • Assist in the creation of rotas and working patterns for all staff to ensure all service is covered with the support of a Care Manager.
  • Coordinate service delivery daily, including managing inquiries from staff, clients, and other health professionals.
  • Undertake formal checks on staff in the field.
  • Undertake initial care assessments and create and update client care plans.
  • Contribute towards the delivery of a high standard of care through effective communication and teamwork.
  • Provide cover delivering care in contingency.

  • Must have a passion for care and a natural caring approach to life.
  • Excellent communication and teamwork skills.
  • Experience in a care setting is preferred.
