Job Title: Case Work Coordinator

We are seeking a highly skilled and dedicated Case Work Coordinator to join our team at Red Snapper Recruitment Limited. As a Case Work Coordinator, you will play a vital role in supporting individuals who have experienced homelessness and have been recently released from prison.

Main Responsibilities:
  • Build and maintain effective relationships with external agencies, including criminal justice services.
  • Progress and track referrals, ensuring successful outcomes for service users.
  • Conduct needs assessments and risk assessments to develop tailored support plans.
  • Provide advice, information, and enabling services to support clients in accessing appropriate services.
  • Coordinate and deliver meaningful occupation opportunities for clients.
  • Maintain accurate and up-to-date client records using the Julian House client record system.
  • Support data analysis and contribute to recommendations for service improvements.
  • Utilize private rented move-on opportunities to ensure effective service pathways.
  • Participate in rostered systems of working, including weekend and evening shifts, and an out-of-hours on-call system.
Person Specification:
  • Understanding of homelessness, social exclusion, and offending.
  • Ability to undertake needs assessments and planning for individuals with complex needs.
  • Deliver a high-quality, person-centered service that takes a strength-based and trauma-informed approach.
  • Ensure client views and feedback influence service design and delivery.
  • Ensure clients are aware of their rights and responsibilities.
  • Meet contract targets, internal KPIs, and compliance requirements.
  • Produce accurate reports and data as requested.
  • Manage incidents and safeguarding, ensuring action is taken and followed up where required.
  • Ensure administration is done promptly and to a high standard using the electronic case management system.
