Key Responsibilities:
  • Manage referrals and assess clients to provide tailored support and guidance.
  • Develop effective relationships with external agencies, including the National Probation Service and Police, to facilitate multi-agency support planning.
  • Track referrals and outcomes for service users, ensuring they receive the necessary support to maintain accommodation and make positive changes in their lives.
  • Complete needs assessments and risk assessments to deliver key elements of support plans that meet individual client needs and reduce re-offending.
  • Provide advice, information, and enabling services to clients, signposting them to appropriate services and supporting their access to these services.
  • Coordinate and deliver meaningful occupation opportunities for clients, promoting their independence and well-being.
  • Maintain accurate and up-to-date client records using the Julian House client record system.
  • Contribute to the analysis of client data, identifying areas of unmet need and recommending changes to services.
  • Utilize private rented move-on opportunities to ensure effective service move-on pathways.
  • Participate in rostered systems of working, including weekend and evening shifts as required, and take part in an out-of-hours on-call system.
Person Specification:
  • Understanding of homelessness, social exclusion, and offending.
  • Ability to undertake needs assessments and planning for individuals with complex needs.
  • Excellent administration skills, with the ability to use electronic case management systems and maintain detailed, concise, and accurate records.
  • Valid driver's license and access to a vehicle.
  • Ability to provide a high-quality, person-centered service that takes a strength-based and trauma-informed approach.
  • Ability to ensure client views and feedback influence the design and delivery of the service.
  • Ability to ensure all clients are aware of their rights and responsibilities.
  • Ability to ensure the service meets targets set out in the contract specification, as well as internal KPIs and compliance.
  • Ability to produce accurate reports and data as requested and at specified intervals.
  • Ability to manage incidents and safeguarding, ensuring action is taken and followed up where required.
