About the Role

We are seeking a skilled Mental Health Practitioner to join our Urgent and Crisis Service team at Sheffield Health and Social Care NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality care to individuals experiencing mental health crises.

Key Responsibilities
  1. Triage urgent/crisis mental health referrals and establish an appropriate response.
  2. Conduct mental health assessments, formulation, and time-limited treatment planning for individuals needing an urgent/crisis response.
  3. Work collaboratively with other disciplines and teams to ensure service users benefit from a multi-disciplinary approach.
  4. Develop strong links and protocols with other services to ensure smooth transitions for patients.
  5. Participate in clinical supervision and contribute to team development.
Requirements
  • Level 2 Social worker or RMN qualification.
  • Crisis experience desirable.
  • UK Driving License desirable.
What We Offer

Working x3 shifts a week (12.5 hrs shifts) allowing a better work-life balance, in addition to unsocial enhancements for shift work. Shifts include twilight, night, and weekend/BH days.

Service Development
  1. Maintain and provide activity information as required.
  2. Contribute towards the development and review of Operational Policies for the service.
  3. Contribute to the team development in relation to training and education.
