**Job Summary**

Framework Housing Association is seeking a highly motivated and compassionate Support Planner to join our dynamic team. As a Support Planner, you will play a vital role in supporting individuals with mental health problems to achieve their goals and improve their overall well-being.

**Key Responsibilities**

  • Plan and provide tailored support to individuals to help them achieve their goals and review their progress.
  • Develop and maintain accurate and relevant support notes, and demonstrate how to resolve problems and advocate for individuals.
  • Communicate effectively with various community agencies and stakeholders to ensure seamless support.
  • Work collaboratively with colleagues to provide a high-quality service and support the development of individuals.

**Requirements**

  • Working knowledge of the support needs of individuals with mental health problems living independently.
  • Ability to demonstrate transferrable skills and a commitment to continuous learning.
  • Strong communication and interpersonal skills, with the ability to work effectively with individuals and teams.
  • Flexibility and creativity in approach, with a strong awareness of professional boundaries.
