Job Title: Complex Care Support Worker

We are seeking a highly skilled and compassionate Complex Care Support Worker to join our team at Claimont Health LTD. As a Complex Care Support Worker, you will play a vital role in providing high-quality care and support to individuals with complex health needs in their own homes.

About the Role:
  • Provide person-centered care and support to individuals with complex health needs
  • Assist with daily living activities, such as personal care, medication management, and mobility
  • Work collaboratively with healthcare professionals to develop and implement care plans
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care delivery
  • Maintain accurate records and reports to ensure continuity of care
Requirements:
  • 1 year or more experience as a Healthcare Assistant with complex care experience
  • NVQ Level 3 related subjects or equivalent
  • NVQ Level 4 related subjects or equivalent
  • Nursery nurse qualifications or similar
  • Diploma in relevant health/social subject
  • Commitment to delivering client-specific care
  • Adaptable and professional approach to support clients and family members
