Job Summary

This exciting new clinical leadership position has been developed to bring together our newly established Virtual Hospital (VH) and Urgent Community Response Services (UCR) for West Essex. These services are an integral part of the West Essex out-of-hospital care model designed to ensure that patients receive rapid, appropriate care and treatment as close to home as possible.

Main Responsibilities
  • Provide high-quality care and leadership to patients, families, and healthcare teams.
  • Work autonomously to assess, diagnose, treat, and evaluate care for adults requiring complex management.
  • Develop and implement strategies to prevent inappropriate hospital admissions and facilitate prompt discharge from acute hospitals.
  • Collaborate with patients, relatives, and partner organizations to establish effective professional relationships.
  • Communicate complex information to patients, carers, and families during the consent process.
  • Authorise decisions to admit patients and/or initiate discharge.
  • Support patients and their families through communicating difficult and highly complex sensitive information.
  • Proactively support patients, carers, and families in coming to terms with their illness/condition.
  • Have a health promotion and prevention approach and comprehensively assess patients for risk factors and early signs of illness.
  • Provide empathy and reassurance through understanding the quality-of-life issues associated with their illness/conditions and treatment-related side effects.
Key Performance Indicators (KPIs)
  • Ensure delivery of core contractual KPIs and targets/expectations for Virtual Hospital/UCR services.
  • Ensure that documentation is of a very high standard, reflective of Advanced Clinical Practice, adhering to local and national guidelines.
  • Identify the skills set and terms of reference required of the Advanced Clinical Practitioner role, ensuring they reflect the individual, holistic needs of patients undergoing care.
Requirements
  • Professional Registration with either NMC, GPhC, or HCPC.
  • MSc in Advanced Practice or equivalent, registered on the Centre of Advancing Practice directory.
  • Possession of a 1st degree in a clinical field, Nursing, Physiotherapy, Paramedic Science, at a minimum 2:2 classification.
  • Digital badge obtained via the Centre of Advancing Practice.
  • Consultation and assessment qualification.
  • Non-Medical Prescribing - v300, if appropriate to registered profession.
  • Significant post-qualification experience (minimum 3 years) in a relevant clinical field.
Desirable Qualifications
  • Post-registration experience/qualification in frailty.
  • Teaching/assessment/mentoring qualification.
Knowledge and Skills
  • Documented evidence of completion of generic advanced practice competencies and a defined range of speciality competencies.
  • Comprehensive understanding of NHS, including role and function of Integrated Care Systems and Primary Care Networks.
  • Comprehensive understanding of Social Care Systems and processes.
  • Advanced knowledge of community service provision.
  • Significant experience gained working within the speciality.
  • Experience of managing complex and competing issues, remaining calm under pressure.
  • Knowledge of clinical governance and risk assessment/management.
