Position: Community Support Specialist

We are seeking a dedicated Community Support Specialist to join our team at Ackerman Pierce Ltd, focusing on ongoing support within the social care sector.

The role will involve working collaboratively with individuals and families to assess their needs and develop tailored support plans. Key responsibilities include:

  • Conducting thorough assessments of health and social care requirements.
  • Creating and implementing detailed support strategies.

  • Utilizing the Decision Support Tool (DST) to determine eligibility for health services.

To be considered for this position, candidates should possess:

  • A minimum of one year of post-qualification experience in the UK social care sector.
  • A valid Enhanced DBS/CRB check (which we can assist in obtaining).
