Key Worker

We are seeking a dedicated and compassionate Key Worker to join our team in providing high-quality support to individuals with mental health needs.
As a Key Worker, you will work closely with our customers to develop and implement personalized support plans, promoting their independence, well-being, and recovery.
You will have a strong understanding of mental health recovery principles and be able to demonstrate a trauma-informed approach to working with customers.
Key Responsibilities- Develop and maintain strong relationships with customers, understanding their needs, goals, and aspirations.
- Conduct regular reviews and assessments to identify areas for improvement and develop targeted support plans.
- Work collaboratively with internal and external stakeholders to ensure seamless delivery of services.
- Provide emotional support, guidance, and advocacy to customers, promoting their autonomy and self-advocacy skills.
- Monitor and report on customer progress, identifying areas for improvement and developing strategies to address these.

Requirements- Relevant qualifications and experience in mental health support or a related field.
