About the Role

We are seeking a highly skilled and compassionate Social Prescribing Link Worker to join our team at Swan Medical Centre. As a key member of our community health team, you will play a vital role in supporting patients to improve their health and wellbeing outcomes.

Key Responsibilities
  • Receiving, assessing, and supporting patients to improve their wellbeing through holistic assessments and co-designed health and wellbeing plans.
  • Working collaboratively with our clinical team to provide high-quality services to patients, sharing ideas, approaches to service delivery, knowledge, and peer support.
  • Promoting referral uptake from within our GP practices and ensuring patients have access to relevant community resources and services.
  • Providing empowering support to communities and adults in a planned and structured way to improve their health, recovery, and wellbeing outcomes.
  • Collating information and data on community resources and organizing them in up-to-date and accessible formats for service users.
Requirements
  • A community, health, social care, counselling, or other relevant experience or professional or academic qualification.
  • Excellent written, verbal, listening, and presentation skills.
  • A proven understanding of safeguarding for children and vulnerable adults and ability to implement relevant policies and procedures.
  • Ability to work autonomously, plan, prioritize work under pressure, and adapt to new models of working.
