About the Role

We are seeking a highly skilled and compassionate Social Prescribing Link Worker to join our team at Swan Medical Centre. As a key member of our healthcare team, you will play a vital role in supporting patients to improve their health and wellbeing outcomes.

Key Responsibilities
  • Receiving, assessing, and supporting patients to improve their wellbeing, and signposting them to relevant services.
  • Working collaboratively with our clinical team to co-design a social prescribing service that meets the needs of our patients and practices.
  • Providing holistic assessments and co-designing personalized social prescribing plans to improve health and wellbeing outcomes for individuals.
  • Managing and prioritizing your own caseload, in accordance with patient needs and priorities.
  • Supporting patients to access other services and promoting social prescribing, its role in self-management, and the wider determinants of health.
Requirements
  • A community, health, social care, counselling, or other relevant experience or professional or academic qualification.
  • Skills and experience of providing empowering support to communities/adults in a planned and structured way to improve their health, recovery, and wellbeing outcomes.
  • Proven skills in collating information and data on community resources and organizing these in up-to-date and accessible formats for a range of different service users from various communities.
  • Excellent written, verbal, listening, and presentation skills.
  • A proven understanding of safeguarding for children and vulnerable adults and ability to implement relevant policies and procedures.
