Social Prescribing Link Worker

We are seeking a highly skilled and compassionate Social Prescribing Link Worker to join our team at Swan Medical Centre. As a key member of our healthcare team, you will play a vital role in supporting patients to improve their health and wellbeing outcomes.
Key Responsibilities- Receiving, assessing, and supporting patients to improve their wellbeing, providing holistic assessments and co-designing personalized health and wellbeing plans.
- Working collaboratively with our healthcare team to co-design a social prescribing service that meets the needs of our patients and practices.
- Providing empowering support to patients, promoting self-management and enabling them to access other services that can support their health and wellbeing.
- Ensuring patients receive basic safeguarding processes and providing opportunities for them to develop friendships, a sense of belonging, and build knowledge, skills, and confidence.
- Promoting social prescribing and its role in self-management and the wider determinants of health.
- Work with patients to undertake GP referrals and self-referrals, conducting holistic assessments and co-designing health and wellbeing plans that identify support needs and ensure maximum engagement in improving health and wellbeing.
- Provide personalized support to patients, ensuring continuity and a coordinated experience of support, remaining a point of contact throughout the individual's social prescription.
- Embed the role and function within our PCN and its practices, ensuring that patients receive relevant information about services available to them.
- Introduce patients to community groups, activities, and statutory services, ensuring they are comfortable and able to engage, and follow up to ensure they are happy and receiving good support.
- Ensure all data, monitoring, and evaluation is kept up to date and reported back as required, highlighting any issues that need resolving.
- A community, health, social care, counselling, or other relevant experience or professional or academic qualification.
- Skills and experience of providing empowering support to communities/adults in a planned and structured way to improve their health, recovery, and wellbeing outcomes.
- Proven skills in collating information and data on community resources and organizing these in up-to-date and accessible formats for a range of different service users from various communities.
- Excellent written, verbal, listening, and presentation skills.
- A proven understanding of safeguarding for children and vulnerable adults and ability to implement relevant policies and procedures.
