Key Responsibilities- Support patients with complex needs through face-to-face appointments and telephone consultations.
- Develop and implement personalized support plans to empower individuals to enhance their overall wellbeing and quality of life.
- Build strong relationships with health and social care professionals to ensure seamless service delivery.
- Work proactively to manage a busy workload and prioritize tasks effectively.
- Confident self-starter with excellent communication and interpersonal skills.
- Previous experience in a similar role or community setting, particularly with vulnerable adults.
