About the Role
As a Social Prescribing Navigator, you will be responsible for:
- Building strong relationships with patients, healthcare professionals, and community organizations to provide holistic support.
- Conducting comprehensive assessments to identify patients' needs and develop personalized support plans.
- Providing emotional support, guidance, and advocacy to patients, empowering them to make informed decisions about their care.
- Collaborating with healthcare professionals to ensure seamless care coordination and referrals.
- Developing and maintaining relationships with community organizations to access resources and services that support patients' needs.
Requirements
To be successful in this role, you will need:
