Access Coordination Officer


Service Care Solutions is seeking a dedicated Access Coordination Officer to join our Family Hub Team.


In this role, you will assist the Access Coordination Team Manager in providing exceptional referral management and administrative support to streamline access to services within the Family Hub Network.

This position offers a hybrid work model with competitive compensation.


Main Responsibilities:

As an Access Coordination Officer, your key responsibilities will include:

1. Collaborating with various professionals to ensure optimal advice and understanding of service users' needs.
2. Assessing the current circumstances and requirements of service users to establish a structured approach.
3. Gaining insights into individual needs and negotiating effectively with other professionals to secure the best guidance and service options.

Your primary duty will be to triage all referrals directed to the service, acting as the initial point of contact and first-level gatekeeper for the Family Hub Network.


Requirements:
- Experience in family hubs or children's centers
- Enhanced DBS clearance
