Job Summary

The Care Coordinator will be responsible for planning and organizing activities that promote the physical, emotional, and social wellbeing of our residents. This is an exciting opportunity for a motivated and creative individual to make a real difference in the lives of others.

Key Responsibilities

  • Develop and implement activity plans that cater to the diverse interests and needs of our residents
  • Collaborate with the care team to ensure seamless delivery of services and activities
  • Build meaningful relationships with residents, their families, and colleagues
  • Monitor and evaluate the effectiveness of activities and make recommendations for improvement
  • Stay up-to-date with industry developments and best practices in care coordination

Requirements

  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Highly organized and able to prioritize tasks effectively
  • Passionate about delivering exceptional care and services
  • Relevant qualifications and experience in care coordination or a related field
