About the Role

We are seeking a highly motivated and enthusiastic Mental Health Practitioner to join our Home Treatment Team in Barrow. As a key member of our team, you will provide comprehensive assessments and triage services to individuals in acute mental health distress, supporting them to avoid hospital admission and facilitating early discharge from hospital.

Main Responsibilities
  • Provide evidence-based mental health assessments and signposting to alternative service provision for individuals requiring specialist urgent mental health assessment.
  • Collaborate with GPs, Police, and Acute Hospital colleagues, as well as self-referrals and referrals from carers, to provide comprehensive, collaborative triage services.
  • Develop and implement crisis plans and contingency planning to support individuals in crisis.

Person Specification
  • Registered Mental Health Nurse, Social Worker, or Occupational Therapist.
  • Able to understand the difference between crisis planning and contingency planning and formulate explicit planning in these areas.
  • Knowledge, understanding, and experience of care pathways for psychosis and non-psychosis and relevant interventions.
  • Effective communication with service users, carers, and multidisciplinary teams, with strong customer service and negotiation skills.
