About the Role

We are seeking a highly motivated and enthusiastic Mental Health Practitioner to join our team in Barrow. As a key member of our Home Treatment Team, you will play a vital role in providing comprehensive assessments and triage services to individuals in acute mental health distress.

Working in a dynamic and supportive environment, you will have the opportunity to develop your skills and expertise in mental health assessment, crisis planning, and contingency planning. You will work closely with service users, carers, and multidisciplinary teams to provide high-quality care and support.

Key Responsibilities
  • Provide comprehensive assessments and triage services to individuals in acute mental health distress
  • Develop and implement crisis plans and contingency plans in collaboration with service users and multidisciplinary teams
  • Work closely with service users, carers, and multidisciplinary teams to provide high-quality care and support
  • Participate in multidisciplinary team meetings and contribute to the development of care plans
Person Specification
  • Registered Mental Health Nurse, Social Worker, or Occupational Therapist
  • Ability to understand the difference between crisis planning and contingency planning
  • Knowledge and experience of care pathways for psychosis and non-psychosis
  • Effective communication and customer service skills
